Home care programs help Massachusetts seniors and residents with disabilities who need support services to live safely at home. Home care programs provide personal care assistance, medication monitoring, housekeeping, home health care, adult day care, meal preparation, laundry, transportation, and other services. Home care programs can delay or prevent nursing facility placements.

The Home and Community-Based Services Waiver (HCBS) is a program for low-income Massachusetts residents who qualify for nursing facility or other institutional care but want to live at home. The Waiver allows MassHealth members to get needed health care and support services at home rather than in an institution. The goal of the program is to help frail seniors, people with intellectual disabilities, and young children with autism live safely in their communities, and to prevent or delay institutionalization.
The Kaileigh Mulligan program is a home care program for severely disabled children who require skilled nursing care or are dependent on assistive technology. It does not count parent income or assets in determining financial eligibility. This program allows children to receive care at home that is equal to the level of care they would receive in a nursing home or other medical facility.
The Massachusetts Home Care Program is a program of support services to help Massachusetts seniors live independently in their own homes. To qualify, seniors must meet income limits and show a need for support services. The Massachusetts Home Care Program is for seniors with low to moderate needs, who do not need daily nursing care. Frail seniors who are medically eligible for nursing facility care can get a higher level of service from the Enhanced Community Options Program (ECOP).

MassHealth pays home health agencies to provide home nursing care; home health aide care; and occupational, physical, and speech/language therapy at home to eligible MassHealth members.
MassHealth gives eligible Standard and CommonHealth members funds to hire personal care attendants (PCAs) to assist them at home with activities of daily living (ADLs) and related household tasks.
Elder Service Plans provide frail elders with comprehensive medical and social services so that they can live in their communities instead of in nursing homes. Services are provided at home, at adult day health centers, or other locations. ESP members must agree to get all covered services through their ESP plan. ESP services are free for seniors with MassHealth. Other seniors pay a monthly premium.
Senior Care Options (SCO) is a program for MassHealth-eligible seniors that provides a complete package of health care and social services, including home health services. SCO members must agree to get all covered services through their SCO plan.
The Home Care Assistance Program (HCAP) provides homemaking services for adults with disabilities, age 18 to 59, who need help with tasks such as meal preparation, laundry, and house cleaning. The program helps people with disabilities live on their own and avoid going to a nursing home or other institution.
Supportive housing programs combine housing with support services. Supportive housing programs are for seniors and people with disabilities who need housing and cannot live independently. Housing options include private homes with host families, public and subsidized apartments with on-site services, congregate housing, and assisted living. The Comprehensive Home-Based Medical Care Program provides home-based nursing care, preventive care, homemaking assistance, and life skills assistance to Massachusetts residents living with HIV/AIDS. The program helps people living with HIV/AIDS remain in their homes, stay as healthy as possible, and avoid unnecessary hospitalizations.
