Where do I apply?
To apply for Veterans' Services, you should contact your local Veterans' Services Officer (VTO):
You must apply in person. If you are disabled and cannot go to the office, you can schedule a home visit.
Your local Veterans' Service Officer will give you a written application (Form VS-1) to complete and sign. A copy of the application is available online on the Veterans' Services of Shrewsbury web site:
You should bring all your documentation with you, including your military discharge record, Form DD214. Otherwise, your application cannot be processed. If you do not have Form DD214, your Veterans' Service Officer can help you get it.

What documentation do I need?
To apply for Veterans' Services, you will need the following documents:
- Military Discharge Record, Form DD214.
- Personal and family information including proof of residency (birth certificate, driver's license, marriage certificate, divorce decree/separation papers, Social Security numbers).
- Proof of resources (bank statements, insurance policies, stocks, bonds, car registration, etc.).
- Proof of income (pay stubs, tax returns, pension statements, other receipts showing how much you receive).
- Proof of expenses (property tax bill, rent receipts, mortgage statements, utility bills, food bills, etc.).
Your Veterans' Service Officer will tell you which documents you need and will help you get any documents you don't have.
How long does it take to find out if I am eligible?
If your documents are in order, you will find out if you are eligible the same day you apply. If some documents have to be checked, it may take 2 to 3 days.
How often do I have to reapply?
Your Veterans' Service Officer will meet with you on a regular basis and let you know if you need to bring in new proofs to verify your eligibility.
What should I do if I am denied benefits?
If you are told you are not eligible for Veterans' Services, you have the right to appeal. Contact the Massachusetts Department of Veterans' Services for more information:
