How do I apply?
There are several ways to apply for Prescription Advantage:

You can call 1-800-AGE-INFO (1-800-243-4636) to request an application. People with hearing disabilities should call TTY: 1-877-610-0241.

You can download an application from the Prescription Advantage web site: Prescription Advantage Application

You can write to Prescription Advantage, P.O. Box 15153, Worcester, MA 01615-0153.

You can call or visit your local Council on Aging or SHINE office. Find Your Regional Elder Care Agency.
If you have any questions about the application, you should call Prescription Advantage Customer Service at 1-800-243-4636 (TTY: 1-877-610-0241) during normal business hours (Monday through Friday, 9 AM to 5 PM).
Where do I send the application?
After you complete the application, you must sign and date it and send it, with all required documentation, to:

What documentation do I need?
You must provide proof of your income. If you are under 65, you must also provide proof of your disability. You should send copies of your documents, not the originals.
For proof of income:

Include copies of federal tax returns (with all schedules) for all household members who filed tax returns

Include copies of Social Security 1099 forms or award letters from the Social Security Administration for all household members who get Social Security benefits

If you or other household members do not file tax returns, or if your current income is very different from the income shown on your tax return, you must include proof of income such as W-2 or 1099 forms. A complete list of acceptable proofs is printed on the application.
For proof of disability, you should include one of the following:
- Social Security award letter for SSDI or SSI benefits
- A copy of your Medicare card
- A certificate of blindness from the Massachusetts Commission for the Blind
- A determination of disability from MassHealth
- A written verification of SSDI or SSI benefits signed by an authorized Social Security representative on Social Security letterhead

When can I join Prescription Advantage?

If you are under 65 years old with a disability, you may join the program any time. Your benefits will start on the first day of the month after your application has been approved.

If you are age 65 or older, you may join the program any time. Your benefits will start on the first day of the month after your application is approved.
Note: If you fill out the application before you turn 65, the plan will hold your application and process it when you turn 65 years old.
