You can download an official copy of the DTA SNAP Food Stamp Benefits Application from the DTA Food Assistance web site, by clicking on one of the links below. The application is available in English, Spanish, and Portuguese. There is also a simplified application for elderly applicants.
These applications are in PDF format. You can also download applications in Microsoft Word format from the DTA Food Assistance web site.
The SNAP food stamp application has complete instructions on how to apply, including a list of the proofs you need. After you print out a copy of the application, answer as many questions as you can. Your application does not have to be complete when you send it in.

After the DTA receives your application, they will call you for an interview. If they have any questions about your answers, or if you left anything blank, they will talk to you about it then.
Remember that if you are eligible for SNAP food stamps, your benefits will start from the date the DTA receives your application. You should file your application as soon as possible.
