What is Group Adult Foster Care (GAFC)?

Group Adult Foster Care (GAFC) is a MassHealth program that pays for personal care services for eligible seniors and adults with disabilities who live in GAFC-approved housing. Housing may be a GAFC assisted living residence or other GAFC housing. To qualify, residents must be eligible for MassHealth and need help with at least one daily personal care task such as bathing or dressing.

GAFC only pays for the cost of personal care services and medication management when you live in approved housing. It does not pay housing costs.


Am I eligible?

To be eligible for Group Adult Foster Care, you must:

  • be at least 22 years old
  • be income eligible for MassHealth Standard
  • live in an assisted living facility or other housing that participates in GAFC
  • need daily assistance with one or more personal care tasks such as bathing, dressing, toileting, meal preparation, and/or moving about
  • have a physician’s statement of your condition
  • be screened and approved for the program


What benefits will I get?

If you qualify for Group Adult Foster Care, you will get these benefits:

  • an individual care plan developed by a registered nurse and case manager
  • ongoing monitoring of needs
  • personal care services in your home each day
  • medication management
  • 24-hour access to services
  • with prior approval, adult day health services or home health aide services


How much does it cost?

There is no cost for Group Adult Foster Care for MassHealth clients who meet the eligibility requirements.

Note: GAFC pays for personal care services and medication management. It does not pay for room and board or support services like housekeeping. You must pay for these separately. If you are not getting any other type of rental assistance, you may be eligible for the Supplemental Security Income SSI-G assisted living benefit to help pay these costs.

To apply for Group Adult Foster Care, call the MassHealth Customer Service Center at 1-800-841-2900 or TTY 1-800-497-4648.
